1. Copy of ARN Card (carry original for verification)/AMFI Certificate (employees not having ARN cards in their  name)
2. Copy of PAN Card (carry original for verification)
3. 2 passport-size photographs
4. Duly filled New Registration Form (download from link below)
5. DD for Rs 1500/- payable in Mumbai, favouring National Institute of Securities Markets.
